About the Role
We are seeking a highly motivated and enthusiastic clinician to join our Discharge Team as a Senior Occupational Therapist. As a key member of our team, you will play a vital role in facilitating transfers from hospital and community beds, working in partnership with colleagues across health and social care to reduce length of stay and improve outcomes for patients and carers.
Key Responsibilities
- Undertake holistic patient assessments and triage patients to the right team and services.
- Work in partnership with health, social, and voluntary agencies to facilitate a seamless pathway of care for patients.
- Produce and maintain accurate records, both written and electronic, in a timely manner.
- Abide by all relevant policies, including information governance, confidentiality, and data protection.
- Communicate effectively with patients, carers, and staff as part of effective patient care.
- Lead the team in the absence of the Team Leads.
- Supervise, lead, inform, and delegate staff as appropriate to the needs of the service.
- Apply analytical skills to assess and interpret referrals into the service and plan patients' pathways.
- Demonstrate high-level problem-solving skills to facilitate patient-centered care.
- Participate in the investigation of complaints and safeguarding concerns.
- Ensure all accidents, complaints, and unusual incidents are reported in accordance with policy.
