Job Summary

We are seeking a highly motivated and enthusiastic Occupational Therapist to join our Discharge Team. As a key member of our team, you will work closely with colleagues across health and social care to reduce length of stay and improve outcomes for patients and carers.

Main Responsibilities
  • Undertake holistic patient assessments and triage patients to the right team and services.
  • Work in partnership with health, social, and voluntary agencies to facilitate a seamless pathway of care for patients.
  • Produce and maintain accurate records, both written and electronic, in a timely manner.
  • Communicate effectively with patients, carers, and staff to ensure effective patient care.
  • Lead the team in the absence of the Team Leads and supervise, lead, inform, and delegate staff as appropriate to the needs of the service.

Person Specification

Essential Qualifications: Registered Therapist

Desirable Qualifications: Educated to Degree level or equivalent experience

Essential Skills: Excellent written and verbal communication skills, flexible approach to changing roles, and ability to work as part of a multi-disciplinary team

Essential Experience: Post-registration experience or equivalent, experience of community working, and experience of working in an Acute Setting

Essential Knowledge: Knowledge of complex patients and admission avoidance
