We are seeking a highly skilled District Nurse Team Lead to join our Community Nursing Team. The successful candidate will have a minimum of 2 years post-qualification experience and a strong background in community specialist practice.

As a key member of our team, you will be responsible for managing a small caseload of complex patients and working closely with staff to ensure high-quality assessment and treatment. You will also liaise with other healthcare professionals and agencies to gather information and advise on additional care needs.

The ideal candidate will have advanced levels of assessment and goal planning skills, as well as awareness of current initiatives within the Local and National health economy. They will also possess excellent organisational skills, including time management, negotiating, and decision-making skills.

The salary for this role is estimated at £55,000 - £60,000 per annum, depending on experience.
