We are looking for a highly skilled and experienced District Nurse Team Lead to join our Community Nursing Team. As a key member of our team, you will be responsible for managing a small caseload of complex patients and working closely with staff to ensure high-quality assessment and treatment.

The successful candidate will have advanced levels of assessment and goal planning skills, as well as awareness of current initiatives within the Local and National health economy. They will also possess excellent organisational skills, including time management, negotiating, and decision-making skills.

The ideal candidate will be able to work flexibly to meet the needs of patients and hold 24-hour responsibility for the team caseload. They will also be able to develop the ethos of integrated care and seamless services, ensuring that patient care is delivered to the highest possible standard.

The estimated salary for this role is £55,000 - £60,000 per annum.
