Role Overview

We are seeking a dedicated Family Support Specialist to join our All Age Continuing Care team in Hampshire. This role offers a dynamic opportunity to work with children, young people, and their families, providing non-clinical support for assessment, review, and case management processes.

The ideal candidate will have experience dealing with sensitive and confidential information and be knowledgeable about NHS Continuing Health Care and Funded Nursing Care guidance/policies.

A competitive salary of £25,000 - £35,000 per annum is on offer, depending on experience.

About the Role
  • Provide non-clinical support to the assessment, review, and case management processes for NHS All Age Continuing Care (Children and Young People)
  • Support Senior Nurse Assessor/Practitioners (Band 6) and/or Nurse Assessor/Practitioners (Band 5) in their roles
  • Pre-populate Decision-Support Tool information under supervision
  • Prepare Funding Request Forms under delegated authority
  • Complete non-clinical case management tasks, including liaising with patients/families and updating care management systems
Required Skills and Qualifications
  • Educated to NVQ 3 level or equivalent qualification/experience
  • Knowledge of NHS Continuing Health Care and Funded Nursing Care guidance/policies
  • Demonstrable experience in dealing with the public and sensitive information
  • Experience of Confidentiality and Data Protection Act
