Adult Carer's Support Specialist

We are excited to announce an exciting opportunity for a highly skilled and experienced Adult Carer's Support Specialist to join our team in Fareham.
As a key member of our Adult Community Mental Health Team, you will play a vital role in providing emotional support and practical assistance to carers and families of individuals with severe and enduring mental health needs.
Main Responsibilities- Develop and implement effective care plans, ensuring individual needs are met.
- Work collaboratively with partner agencies to ensure seamless delivery of care and support services.
- Maintain accurate records and reports, ensuring compliance with NHS standards and regulations.
To be successful in this role, you will need:
- Literacy and numeracy skills.
- Previous experience in care or a related field.
- Basic computer skills and ability to maintain accurate records.
- A valid driving license and access to a suitable vehicle for work purposes.
The salary for this role is £24,071 - £25,674 per annum, based on full-time hours. We offer a range of benefits, including opportunities for career development, training, and collaborative working.
