About the Role

We are seeking a Young Adult Transition Specialist to join our team, providing support to three young adults with autism and ADHD as they transition from child to adult services. The successful candidate will have excellent communication skills, be able to work independently, and have experience in supporting individuals with complex needs.

The key responsibilities of this role include providing one-to-one and group support, promoting independence and self-advocacy skills, and encouraging participation in activities such as going out, hobbies, and education. The specialist will also collaborate with multidisciplinary teams to develop and implement support plans.

Requirements

- A minimum of 1 year's experience in supporting individuals with complex needs
- Ability to work independently and as part of a team
- Excellent communication and interpersonal skills
- Ability to adapt to changing situations and priorities
