NHS Community Healthcare Specialist

Annual salary range: £46,148 - £52,809.

This position involves providing expert care and guidance to patients and their families in the comfort of their own homes.

As an NHS Community Healthcare Specialist, you will work alongside a multidisciplinary team to deliver person-centered care and address the physical and emotional needs of patients.

  • Prioritize and manage your workload effectively to meet deadlines and respond promptly to changing circumstances.
  • Maintain accurate records and documentation, adhering to NHS standards and guidelines.
  • Develop and maintain strong relationships with healthcare professionals, patients, and their families to ensure a smooth transition of care.

An independent prescriber with experience in working with patients with long-term conditions and mental well-being would be an excellent fit for this role.
