Job Description

The successful candidate will be responsible for providing expert interventions and rehabilitation as part of a dynamic, tight-knit team that reacts quickly when people are in crisis. Key duties will include:

  • Providing comprehensive specialist assessment, review, case management, and evaluation of patient needs
  • Planning programs of care to promote health gain and maximize independence
  • Allocating workloads to members of the Rapid Response & Intermediate Care (RRIC) team
  • Collaborating with various professionals to ensure timely delivery of care

The ideal candidate will possess excellent clinical skills, leadership abilities, and strong communication skills. They will also have experience working in a community or in-patient setting and be able to manage complex cases.

Required Skills and Qualifications

To be considered for this role, applicants must meet the following essential criteria:

  • Holding a valid and current driving license and access to a car
  • Registered Allied Healthcare Professional with HCPC registration
  • Degree/Diploma in Physiotherapy or Occupational Therapy
  • Experience of recent Continuous Professional Development

Salary Information

This role comes with a competitive salary package, reflecting the importance of this position in our organization. The estimated salary for this role is between £42,000-£50,000 per annum, depending on experience and qualifications.
