Job Description

At Buckinghamshire Healthcare NHS Trust, we are committed to providing outstanding care and support to our patients. We are currently seeking a highly skilled and experienced Staff Nurse to join our Adult Community Healthcare Team.

This is an exciting opportunity to work in a dynamic and fast-paced environment, where you will have the chance to make a real difference in the lives of our patients. As a member of our team, you will be responsible for delivering high-quality nursing interventions to patients in their own homes, working closely with other healthcare professionals to ensure seamless care.

We are looking for an individual who is passionate about community healthcare and has a strong commitment to delivering excellent patient outcomes. You should have recent experience working in a community setting and possess strong clinical assessment and planning skills. A valid driving license is also essential for this role.

Key Responsibilities:

  • To assess the physical and psycho-social needs of patients and develop tailored care plans to meet their individual requirements.
  • To deliver high-quality nursing interventions, including therapeutic assessments and interventions, in accordance with best practice guidelines.
  • To work collaboratively with other healthcare professionals to ensure comprehensive care and support for patients.
  • To maintain accurate and up-to-date records of patient interactions and progress.

Requirements:

  • A current registration as a Registered Health Professional (RHP) with the relevant professional body.
  • A minimum of 1 year's experience working in a community setting, preferably in a similar role.
  • Strong clinical assessment and planning skills, with the ability to think critically and creatively.
  • A valid driving license and access to a car.

Salary: £30,000 - £45,000 per annum (dependent on experience), plus benefits package including annual leave entitlement, pension scheme, and access to NHS discount schemes.
