Job Description

We are seeking a skilled and compassionate Community Support Worker to join our team at Central London Community Healthcare NHS Foundation Trust. As a Community Support Worker, you will play a vital role in providing high-quality care and support to individuals in their own homes or community settings.

Key Responsibilities
  • Deliver person-centered care and support to individuals with complex needs, promoting independence and well-being.
  • Work collaboratively with multidisciplinary teams, including healthcare professionals, social workers, and family members, to develop and implement personalized care plans.
  • Conduct thorough assessments of individuals' needs, identifying areas for improvement and developing strategies to address these.
  • Provide emotional support and guidance to individuals, promoting confidence and self-esteem.
  • Maintain accurate and comprehensive records of individuals' care and support, ensuring confidentiality and data protection.
  • Participate in regular team meetings and contribute to the development of care plans and strategies.
Requirements
  • Hold a relevant qualification in Health and Social Care, such as an NVQ 3 or equivalent.
  • Have experience working in care and rehabilitation settings, with a focus on supporting individuals with complex needs.
  • Demonstrate excellent communication and interpersonal skills, with the ability to build strong relationships with individuals, families, and colleagues.
  • Be able to work independently and as part of a team, with a flexible approach to working hours and locations.
  • Hold a valid driving license and have access to a car for business purposes (unless exempt).
Desirable Criteria
  • Experience working with electronic record systems and software.
  • Knowledge of enablement strategies and the role of the support worker in promoting independence.
  • Understanding of the Trust's values and commitment to delivering high-quality care and support.
