About the Role

Key Responsibilities
  • Deliver high-quality patient care and support in a community setting, working closely with healthcare professionals to develop and implement personalized care plans.
  • Conduct thorough assessments of patients' needs and develop strategies to promote independence, mobility, and overall well-being.
  • Provide emotional support and guidance to patients and their families, promoting a positive and empowering experience.
  • Collaborate with multidisciplinary teams to ensure seamless care coordination and effective communication.
  • Maintain accurate and detailed records of patient interactions, progress, and outcomes, adhering to strict confidentiality and data protection protocols.
Requirements
  • NVQ 3 in Health or Social Care or equivalent academic qualification.
  • Experience in care work, preferably in a health or social care setting.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with patients, families, and healthcare professionals.
  • Self-management skills, including time management and workload prioritization.
  • Ability to analyze, interpret, and record data in a clear and succinct manner.
  • Good analytical and problem-solving skills, with the ability to work under pressure and manage competing demands.
