Community Support Worker

We are seeking a highly skilled and compassionate Community Support Worker to join our team at Central London Community Health Trust. As a Community Support Worker, you will play a vital role in supporting patients to regain their independence and live fulfilling lives.
Key Responsibilities- Deliver high-quality patient care and support in a community setting, working closely with healthcare professionals to develop and implement personalized care plans.
- Conduct thorough assessments of patients' needs and develop strategies to promote independence, mobility, and overall well-being.
- Provide emotional support and guidance to patients and their families, promoting a positive and empowering experience.
- Collaborate with multidisciplinary teams to ensure seamless care coordination and effective communication.
- Maintain accurate and detailed records of patient interactions, progress, and outcomes, adhering to strict confidentiality and data protection protocols.
- NVQ 3 in Health or Social Care or equivalent academic qualification.
- Experience in care work, preferably in a health or social care setting.
- Excellent communication and interpersonal skills, with the ability to build strong relationships with patients, families, and healthcare professionals.
- Self-management skills, including time management and workload prioritization.
- Ability to analyze, interpret, and record data in a clear and succinct manner.
- Good analytical and problem-solving skills, with the ability to work under pressure and manage competing demands.
