Job Description: Community Health Nurse - Unplanned Care

About the Role:

We are seeking a skilled and dedicated Community Health Nurse to join our Unplanned Care Team at Central London Community Healthcare NHS Trust. As a key member of our team, you will be responsible for delivering high-quality care to patients in their own homes.

Key Responsibilities:

  1. Utilize advanced clinical and diagnostic skills to carry out thorough assessments of physical and psycho-social needs.
  2. Develop comprehensive short-term care plans to optimize health and wellbeing, preventing hospital attendance or allowing early discharge home.
  3. Work within Clinical Guidelines, Policies, Procedures, and Patient Group Directions (PGD's) while exercising expert judgment.
