We are seeking a highly motivated Social Care Professional to join our team, working with adults with learning disabilities. This is a permanent role offering a salary of £24,170 to £27,313 per annum, with hybrid working and 32 days annual leave.

In this role, you will work alongside social workers to support adults with learning disabilities, helping them achieve their goals and live independently. Your key responsibilities will include:

  • Conducting assessments and undertaking care reviews to identify individual needs and develop tailored care plans
  • Collaborating with other professionals to ensure seamless service delivery and high-quality outcomes
  • Participating in a duty rota to respond to incidents, safeguarding alerts, and information requests

To succeed in this role, you will need:

  • Experience in social care, preferably working with adults with learning disabilities
  • Excellent communication and interpersonal skills
