We are seeking a skilled Home-Based Patient Support Professional to join our team in providing high-quality care and support to patients in their own homes. This is a rewarding opportunity to make a real difference in the lives of others.

Main Responsibilities
  • To provide care and rehabilitation for service users within their own homes under the clinical direction of qualified staff.
  • To support multi-disciplinary health professionals in providing a comprehensive rehabilitation programme to prevent avoidable hospital admission.
  • To maintain accurate personal and professional records adhering to trust policies and procedures.
About the Opportunity

This role offers a competitive salary of £25,000 - £31,000 per annum, depending on experience, plus benefits including opportunities for career progression and full training provided.
