Pennine Care NHS Foundation Trust is seeking a highly skilled Community Intervention Lead to join our team.

The successful candidate will be responsible for delivering recovery-focused interventions to adults in mental health crisis, managing risk, and making clear decisions about care planning and intervention.

As a key member of the multidisciplinary team, you will work autonomously in the community, complete full mental health assessments, and manage risk while being an integral part of the team.

Key ResponsibilitiesRecovery-Focused Interventions

The successful candidate will be responsible for delivering recovery-focused interventions, assessing risks, and making clear decisions about care planning and intervention.

Mental Health Assessments

You will complete full mental health assessments, evaluating and recording outcomes, and adapting care based on these outcomes.

Salary

The salary for this role is £42,000 - £47,000 per annum, depending on experience.
