Job Summary

We are seeking a dedicated Health and Wellbeing Coach to provide person-centered support to individuals serving community sentences or transitioning from prison in Durham and Cleveland.

Working within the new Dynamic Probation Framework, you will deliver structured support, helping individuals address personal wellbeing, mental health, and social challenges to achieve their goals and make a positive return to their communities.

Key Responsibilities
  • Provide 1:1 and group-based support to service users, creating individualised action plans and working towards achievable goals.
  • Work collaboratively in a multi-agency setting to support successful outcomes for service users.
  • Foster a safe, supportive environment, using a coaching approach to empower service users.
  • Conduct assessments, maintain records, and meet performance targets within agreed timeframes.
  • Support service users in overcoming barriers, developing life skills, and accessing resources such as housing and health services.
  • Participate in out-of-hours support via weekday helpline (5-8 pm) and occasional Saturday drop-in sessions.
  • Uphold Rethink's values, ensuring a respectful and inclusive approach to all individuals.
Qualifications and Experience
  • NVQ Level 3 or equivalent in Health and Social Care, Mental Health Coaching, or a related field.
  • Experience supporting vulnerable individuals using structured, time-limited support in a 1:1 and/or group setting.
  • Knowledge of mental health, criminal justice, housing, and substance misuse.
  • Strong communication skills and an understanding of cultural sensitivity.
  • Full UK driving license and access to own transport.
Desirable
  • Lived experience with mental health or criminal justice issues.
  • Experience within prison environments or with offender support.
  • Training in health education (e.g., Mental Health First Aid).
Personal Qualities
  • Empathetic, patient, and resilient in working with diverse individuals.
  • Confident in managing boundaries and maintaining professional relationships.
  • Skilled in time management, able to prioritize and adapt to changing demands.
