About the Role

We are seeking a highly motivated and compassionate Housing Support Worker to join our team in Wembley. As a key member of our team, you will provide housing-related support to vulnerable adults and those from complex backgrounds, empowering them to achieve independent living and improve their skills, resilience, and pathways to independence.

Key Responsibilities
  • Manage a caseload of participants with diverse needs, providing face-to-face support and contributing to the development of support plans, risk assessments, and reviews.
  • Create and implement SMART support plans in collaboration with participants, promoting their autonomy and self-reliance.
  • Participate in and encourage participant involvement in various projects, activities, and initiatives, fostering a sense of community and social connection.
  • Support the service and team to meet key KPIs and objectives, ensuring the highest standards of care and support.
  • Recognize support needs in individuals and initiate appropriate interventions to prevent further deterioration, promoting a culture of early intervention and prevention.
  • Complete safeguarding and other referrals as required, ensuring the well-being and safety of participants.
  • Ensure participants understand their rights and responsibilities, providing access to the right tools and resources to support their reintegration into society and building networks.
  • Provide advice, information, guidance, and life skills training to participants, empowering them to make informed decisions and take control of their lives.
  • Develop and maintain strong relationships with internal and external stakeholders, including community partners, local authorities, and service providers, to ensure seamless support and referrals.
