Senior Community Mental Health Practitioner

About the Role
We are seeking a skilled and compassionate Community Psychiatric Nurse to join our team at Kent and Medway NHS and Social Care Partnership Trust. As a Community Psychiatric Nurse, you will play a vital role in providing mental health care to older adults in our community.
Responsibilities
Key responsibilities include conducting initial assessments of individuals with memory and mental health needs, managing a caseload of clients, and participating in team meetings and multidisciplinary discussions. Additionally, you will be responsible for supporting and supervising junior colleagues, as well as taking on a lead role within the team as needed.
Requirements
Applicants should possess a relevant formal vocational qualification and first-level nursing registration. A minimum of 6 months post-registration experience is also desirable. Furthermore, good IT skills, the ability to travel across the locality in a timely manner, and experience in or knowledge around mental illness and dementia are essential.
