Community Health Practitioner

**Job Overview:**
We are currently recruiting for a Community Health Practitioner - Vulnerable Children Team to join our dedicated team at Kent Community Health NHS Foundation Trust. This is an exciting opportunity to make a real difference in the lives of vulnerable children and their families.
As a key member of our community health service, you will work collaboratively with other professionals to deliver targeted interventions and promote healthy lifestyles within our local population. The successful candidate will be responsible for conducting universal mandated checks, packages of care, and working closely with partner agencies to identify and protect vulnerable children.
Key Requirements:
- Degree-level qualification in a relevant field (e.g., nursing, midwifery, social work).
- Experience working with vulnerable children and families.
- Excellent communication and interpersonal skills.
