About the Role

We are seeking an experienced and dedicated Community Nurse to join our team at Kent Community Health NHS Foundation Trust. As a Community Nurse, you will provide high-quality patient care and work closely with our multidisciplinary team to deliver excellent services to our communities.

Main Responsibilities

Key tasks and responsibilities of the role include:

  1. Providing expert patient care and support to patients in their own homes or community settings.
  2. Working closely with our multidisciplinary team to deliver a high-quality service and achieve excellent patient outcomes.
  3. Developing and maintaining strong relationships with patients, families, and carers to promote patient-centered care.
  4. Collaborating with other healthcare professionals to ensure seamless care transitions and effective communication.
  5. Participating in quality improvement initiatives and contributing to the development of our services.

Requirements and Qualifications

To be considered for this role, you will need:

  1. A registered health care professional qualification (NMC registration).
  2. Experience in a community nursing setting, with a strong understanding of the complexities of community-based care.
  3. Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and staff at all levels.
  4. A valid driving license and access to a car for work purposes.
