Job Title: Stroke Rehabilitation Unit Leader

We are seeking a highly skilled and experienced Advanced Clinical Practitioner to join our Stroke Rehabilitation Unit as a leader and key member of our multidisciplinary team.

Main Responsibilities:
  • Develop and lead the rehabilitation beds on the stroke rehabilitation unit, ensuring high-quality patient care and effective discharge planning.
  • Work collaboratively with our Consultant Physiotherapist to facilitate a coordinated service for patients with stroke, across the entire stroke pathway.
  • Maintain and develop relationships with external partners to optimize transitions of care and ensure seamless patient flow.
  • Be accountable for the delivery of stroke rehabilitation services, ensuring they meet the highest standards of quality and patient safety.
