Job Title: Stroke Rehabilitation Unit Leader

We are seeking a highly skilled and experienced Advanced Clinical Practitioner to join our Stroke Rehabilitation Unit as a leader and key member of our multidisciplinary team.

Main Responsibilities:
  • Develop and lead the rehabilitation beds on the stroke rehabilitation unit, ensuring high-quality patient care and effective transitions of care.
  • Work collaboratively with our Consultant Physiotherapist and other healthcare professionals to deliver a coordinated service for patients with stroke and those requiring rehabilitation.
  • Maintain and develop relationships with external partners to optimize transitions of care and ensure seamless patient flow.
  • Be accountable for the delivery of stroke rehabilitation services, ensuring they meet the highest standards of quality and safety.

We are looking for a suitably qualified and experienced Advanced Clinical Practitioner who is passionate about delivering high-quality patient care and rehabilitation services.
