Palliative Care Coordinator

We are seeking a highly skilled and compassionate Palliative Care Coordinator to join our team at Lawrence Nurses. As a key member of our nursing team, you will play a vital role in providing holistic care to patients and their families.
Key Responsibilities- Coordinate new patient referrals and undertake assessments to determine the level of care required.
- Act as a central point of contact for patients, families, and carers, providing emotional support and guidance throughout their care journey.
- Work collaboratively with our nursing team to coordinate care, allocate resources, and ensure seamless delivery of services.
- Provide clinical leadership and manage patient caseloads, ensuring that each patient receives the highest standard of care.
- Communicate effectively with patients, families, and carers, using a sensitive and empathetic approach to address their needs and concerns.
- Work autonomously, using your initiative and judgment to resolve complex issues and make informed decisions.
