Palliative Care Coordinator

Lawrence Nurses is seeking a highly skilled and compassionate Palliative Care Coordinator to join our dynamic team. As a key member of our nursing team, you will play a vital role in providing exceptional end of life care to patients and their families.
Key Responsibilities- Coordinate new patient referrals and undertake assessments to determine the level of nursing support required.
- Act as a central point of contact for patients, families, and carers, providing emotional support and guidance throughout the care journey.
- Manage patient caseloads, including active, pending, and RIP patients, ensuring seamless care coordination.
- Support Lawrence Nurses in providing holistic care to patients in their homes, working closely with the Registered Nurse Manager as needed.
- Provide clinical leadership and guidance to the nursing team, ensuring high-quality care standards are met.
- Develop and maintain strong relationships with community nurses, healthcare professionals, and other stakeholders to ensure comprehensive care.
- Manage patient risk assessments, ensuring the safety and well-being of patients and staff.
- Support patients, families, and carers in their care planning, ensuring their needs are met and their voices are heard.
- Collaborate with the nursing team to develop and implement care plans, ensuring patient-centered care.
- Maintain accurate records and documentation, ensuring compliance with regulatory requirements.
- Participate in ongoing professional development, staying up-to-date with best practices in palliative care.

Person Specification- RN Registration Level 1 or 2, current NMC registration with relevant post-registration qualification, ENB 931 or Diploma in Palliative care or equivalent.
- Minimum of two years recent experience in palliative care, either in the NHS or other relevant care setting.
- Recent experience of working in the community.
- Up-to-date awareness of the skills and knowledge required for patients with palliative care needs.
- Ability to communicate effectively with people in a diplomatic and tactful manner.
- Ability to work alone in a complex and stressful environment, demonstrating appropriate coping mechanisms.
- Ability to quickly identify and solve problems, taking action to address issues.
- Ability to use own initiative and preparedness to seek advice when necessary.
- Effective communication and interpersonal skills.
