About the Role

Lawrence Nurses is seeking a skilled and compassionate Palliative Care Nurse Coordinator to join our dynamic team. As a key member of our nursing team, you will play a vital role in providing holistic and patient-centred care to individuals at the end of their life.

Key Responsibilities
  • Be responsible for new patient referrals and undertaking assessments to determine the level of care required.
  • Act as a central point of contact for patients, families, and carers, providing emotional support and guidance throughout their care journey.
  • Coordinate and allocate nursing staff to provide care and support to patients in their homes, working closely with the Registered Nurse Manager as needed.
  • Provide clinical leadership and manage patient caseloads, ensuring that patients receive high-quality care and support.
  • Work autonomously, using your skills and knowledge to make sound judgments and communicate effectively with patients, families, and carers.
  • Offer end of life care and symptom control, working closely with other healthcare professionals to ensure that patients receive comprehensive care.
  • Provide 24-hour on-call assistance to support nursing staff and offer support to patients' families and carers between 8am and 10pm.
  • Respond quickly to urgent on-call contacts, which may involve travel to patients' homes.
