About the Role

We are seeking a highly skilled and compassionate Palliative Care Nurse Coordinator to join our dynamic team at Lawrence Nurses. As a key member of our nursing team, you will play a vital role in providing holistic care to patients and their families, ensuring their needs are met and their dignity is respected.

Key Responsibilities
  • Be responsible for new patient referrals and undertaking assessments to determine the level of care required.
  • Act as a central point of contact for patients, families, and carers, providing emotional support and guidance throughout their care journey.
  • Coordinate and allocate nursing staff to provide care to patients in their homes, ensuring seamless transitions and continuity of care.
  • Provide clinical leadership and manage patient caseloads, ensuring that patients receive the highest standard of care.
  • Work autonomously, using your expertise and judgment to make decisions and take action to address issues.
  • Offer end of life care and symptom control, working closely with patients, families, and carers to ensure their needs are met.
  • Provide 24-hour on-call assistance to support nursing staff and respond to urgent contacts.
  • Support patients' families and carers between 8am and 10pm, providing emotional support and guidance.
