About the Role

Lawrence Nurses is seeking a highly skilled and compassionate Palliative Care Coordinator to join our dynamic team. As a key member of our nursing team, you will play a vital role in providing exceptional end of life care to patients and their families.

Key Responsibilities
  • Coordinate new patient referrals and undertake assessments to determine the level of nursing support required.
  • Act as a central point of contact for patients, families, and carers, providing emotional support and guidance throughout the care journey.
  • Manage patient caseloads, including active, pending, and RIP patients, ensuring seamless care coordination.
  • Support Lawrence Nurses in providing holistic care to patients in their homes, working closely with the Registered Nurse Manager as needed.
  • Provide clinical leadership and guidance to the nursing team, ensuring high-quality care standards are met.
  • Develop and maintain strong relationships with community nurses, healthcare professionals, and other stakeholders to ensure comprehensive care.
  • Manage patient risk assessments, ensuring timely interventions and effective care planning.
  • Support patients, families, and carers through the care journey, providing emotional support and guidance as needed.
  • Maintain accurate records and documentation, ensuring compliance with regulatory requirements.
  • Participate in ongoing professional development, staying up-to-date with best practices and industry developments.
Requirements
  • Registered Nurse (Level 1 or 2) with current NMC registration and relevant post-registration qualification.
  • Minimum two years' experience in palliative care, preferably in a community setting.
  • Excellent communication and interpersonal skills, with the ability to work effectively in a team environment.
  • Strong problem-solving and critical thinking skills, with the ability to make sound judgments.
  • Ability to work autonomously, with a high level of independence and self-motivation.
  • Basic IT skills, with the ability to access data and communicate effectively via email.
