Lawrence Nurses is seeking a highly skilled and compassionate Palliative Care Coordinator to join our dynamic team. As a key member of our nursing team, you will play a vital role in providing exceptional end of life care to patients and their families.
Key Responsibilities- Coordinate new patient referrals and undertake assessments to determine the level of nursing support required.
- Act as a central point of contact for patients, families, and carers, providing emotional support and guidance throughout the care journey.
- Manage patient caseloads, including active, pending, and RIP patients, ensuring seamless care coordination.
- Support Lawrence Nurses in providing holistic care to patients in their homes, working closely with the Registered Nurse Manager as needed.
- Provide clinical leadership and guidance to the nursing team, ensuring high-quality care standards are met.
- Develop and maintain strong relationships with community nurses, healthcare professionals, and other stakeholders to ensure comprehensive care.
- Manage patient risk assessments, ensuring timely interventions and effective care planning.
- Support patients, families, and carers through the care journey, providing emotional support and guidance as needed.
- Maintain accurate records and documentation, ensuring compliance with regulatory requirements.
- Participate in ongoing professional development, staying up-to-date with best practices and industry developments.
- Registered Nurse (Level 1 or 2) with current NMC registration and relevant post-registration qualification.
- Minimum two years' experience in palliative care, preferably in a community setting.
- Excellent communication and interpersonal skills, with the ability to work effectively in a team environment.
- Strong problem-solving and critical thinking skills, with the ability to make sound judgments.
- Ability to work autonomously, with a high level of independence and self-motivation.
- Basic IT skills, with the ability to access data and communicate effectively via email.
