Job Purpose

We are seeking a skilled Community Outreach Specialist to join our team in West Kent. As a Community Outreach Specialist, you will work with clients to deliver mental health and wellbeing interventions in a case management approach. You will also facilitate and support clients with complex and/or multiple barriers to success in health and social care settings.

Key Responsibilities

  • Deliver mental health and wellbeing interventions in a case management approach
  • Facilitate and support clients with complex and/or multiple barriers to success in health and social care settings
  • Work with clients to achieve their goals and improve their wellbeing

Requirements

  • Minimum of SVQ/NVQ level 3
  • Experience of delivering mental health and wellbeing interventions in a case management approach
  • Experience of facilitating and supporting clients with complex and/or multiple barriers to success in health and social care settings
