Job Purpose
We are seeking a Community Support Specialist to join our team in West Kent. As a Community Support Specialist, you will play a vital role in delivering mental health and wellbeing interventions in a case management approach. You will work with clients with complex and/or multiple barriers to success in health and social care settings.
Key Responsibilities
- Deliver mental health and wellbeing interventions in a case management approach
- Facilitate and support clients with complex and/or multiple barriers to success in health and social care settings
- Work collaboratively with the community team to provide person-centred support aimed at delivering wellbeing improvements and community connectedness
Requirements
- Minimum of SVQ/NVQ level 3
- Experience of delivering mental health and wellbeing interventions in a case management approach
- Experience of facilitating and supporting clients with complex and/or multiple barriers to success in health and social care settings
