Community Support Worker Lead

We are seeking an experienced Home Based Social Worker to join our Home First Neighbourhood Team at Ackerman Pierce Ltd. This is an exciting opportunity to make a positive impact on individuals' lives, helping them to regain independence and live safely in their own homes.
Your Key Responsibilities:
- Conduct thorough assessments to identify individuals' needs and develop personalized plans
- Work collaboratively with healthcare professionals, reablement providers, and the voluntary sector to ensure comprehensive support
- Utilize your expertise in activity analysis, assistive technology, and minor adaptations to optimize individuals' independence and reduce formal care requirements
Requirements:
- Valid driver's license and access to a vehicle
- Exceptional communication and collaboration skills
- Empathetic and non-judgmental approach towards individuals from diverse backgrounds and needs
