Job Summary:

Creative Support is seeking an experienced practitioner with management experience to provide care and support to individuals with learning disabilities. As a Supported Living Coordinator, you will play a vital role in developing the independent living skills and community inclusion of service users.

Key Responsibilities:

  • Develop and implement personalized care plans to promote valued and meaningful lifestyles for service users.
  • Collaborate with service users, families, and the multi-disciplinary team to maximize service user outcomes.
  • Provide respectful personal care, emotional, and practical support to service users.
  • Supervise and support staff to best meet service user needs and preferences.
  • Implement Creative Support principles, policies, procedures, and guidelines to ensure the service meets CQC requirements.
  • Support service users with behavior that challenges services and implement positive behavior support.

Requirements:

  • Experience in providing care and support to individuals with learning disabilities.
  • Management experience and ability to lead a team.
  • Access to a car.
  • Flexibility to work evenings and weekends.
