About the Job

Somerset NHS Foundation Trust is seeking an experienced nurse to join our Homeless Rough Sleepers Nursing Team in Mendip. This is a challenging yet rewarding position that requires flexibility and adaptability, enabling you to make a real difference in the lives of vulnerable individuals.

As a key member of our team, you will play a vital front-line role tackling health inequalities by working with one of the most vulnerable groups in the county. You will require excellent communication skills, a teamwork approach, and the ability to think outside the box to provide high-quality nursing care, as well as a full driving license and vehicle use.

Key Responsibilities
  1. Work collaboratively with people and partner agencies to proactively engage with individuals at various venues in local communities, helping people improve their lifestyle and overall health.
  2. Provide appropriate and compassionate healthcare, contributing to a holistic approach addressing wider determinants of health.
  3. Work with the clinical lead to ensure the team develops to meet identified needs and undertake relevant training to enable autonomous competent practice.
  4. Making every contact count, taking every opportunity to impact directly and indirectly on health and wellbeing.
