We are excited to announce a new opportunity for a skilled professional to join our Somerset Hub for Coordinating Care team as a Patient Services Facilitator. This role will primarily be based within the Somerset Primary Link Team, with the possibility of working across teams.

In this role, you will have the opportunity to make a real difference in the lives of patients and their families. You will work closely with the healthcare team to ensure that patients receive the best possible care, while also providing administrative support as needed.

Your Key Responsibilities:
  • Provide administrative support to the healthcare team.
  • Coordinate patient care activities and maintain accurate records.
  • Communicate effectively with patients, families, and healthcare staff.
