We are seeking a dedicated Community Wellbeing Coordinator to join our team at Somerset NHS Foundation Trust. The successful candidate will work with individuals who have severe and enduring mental health problems, including those with coexisting substance misuse disorders.

Job Responsibilities

The postholder will carry a clinical caseload and will provide supervision and advice to other mental health staff in relation to their working practice with this service user group.

This is an excellent opportunity for a qualified healthcare professional to develop their skills and expertise in working with older adults and individuals with complex clinical presentations.

About the Role:

  • To act as a key worker for a caseload of individuals, assessing their needs, and developing their recovery care plan, coordinating and reviewing their care and treatment outlined in the Dialog plus.
  • To develop and maintain one's own clinical skills and expertise in the area of approaches to, and treatment of people with a mental health difficulty, Dual Diagnosis, and to be reflective and responsive to meaningful service user and carer involvement.

Requirements:

  • Experience of offering clinical supervision to other professionals.
  • Working with clients in the community.
