About the Role

We are seeking a highly motivated and compassionate Community Care Professional to join our Ipswich Adult community team. As a key member of our team, you will work closely with service users, their families, and external agencies to provide holistic care and support.

Key Responsibilities:
  • Develop and implement effective care plans, ensuring comprehensive support for individuals with mental health needs.
  • Collaborate with healthcare professionals, social workers, and other stakeholders to ensure seamless care coordination.
  • Provide emotional support and guidance to service users and their families, promoting independence and empowerment.
Salary and Benefits:

This role offers a competitive salary of £**£35,000-£45,000 per annum**, depending on experience, as well as a range of benefits, including an NHS pension scheme, comprehensive training programs, and flexible working arrangements.

In addition to your salary, you will also have access to a generous annual leave entitlement, starting at 27 days per annum, increasing to 33 days per annum based on length of service.
