Care Coordinator Specialist

Pennine Care NHS Foundation Trust is seeking an experienced Care Coordinator to join our Early Intervention Team. As a key member of the team, you will work closely with mental health nurses, social workers, CBT therapists, and support workers to deliver high-quality care to individuals experiencing first episode psychosis.
The ideal candidate will have a strong background in mental health nursing, occupational therapy, or social work, with substantial experience of working in a multi-disciplinary team setting. You will be responsible for managing a defined caseload, contributing to duty and clinic work, and participating in team development issues.
You will work closely with patients, carers, and other healthcare professionals to assess, plan, and implement care that meets the needs of individuals with complex mental health problems. A good working knowledge of therapeutic assessment techniques and interventions is essential, as well as a comprehensive understanding of Safeguarding adults procedures and practice.
