Care Coordinator Specialist

We are seeking an experienced Care Coordinator to join our team at Cheadle Medical Practice. This is a fixed-term position, ideal for individuals with experience in safeguarding and a passion for delivering high-quality administrative support.
Job Description- Support key safeguarding activities across the practice, working closely with safeguarding leads to ensure efficient and effective delivery of objectives.
- Provide administrative and operational support to clinicians and managers, utilizing strong organizational skills and attention to detail.
- Maintain accurate records and registers, ensuring ongoing review and updates to safeguarding flags.
- Pull together relevant information for child protection case conferences, safeguarding adults processes, and other statutory reviews.
- Organize MDT meetings and facilitate information sharing between GP safeguarding leads, health visitors, school nurses, and midwives.
Safeguarding coordination and administration, training and development, audit and quality assurance, and embedding learning from safeguarding reviews.
Person Specification- Achieved grade C or above in English and Maths GCSE or equivalent.
- NVQ Level III in Health and Social Care.
- Previous experience working with the general public, safeguarding policies, and procedures within primary care.
- Good IT skills, proficient in Microsoft packages, and ability to use databases and produce reports.
