Job Title: Care Coordinator

About the Role:

The successful candidate will be responsible for acting as a key worker, responding to patient behaviours, formulating risk assessments and care plans, managing medication, and maintaining patient and staff safety.

  • Excellent communication and IT skills

The estimated salary for this position is £32,000 - £40,000 per annum, depending on experience.
