Job Title: District Nurse Team Manager

Job Summary:

Livewell Southwest is seeking an experienced District Nurse Team Manager to lead our community nursing team in Tavistock. As a key member of our team, you will be responsible for the operational management of the team, ensuring the delivery of high-quality patient care and services.

Main Responsibilities:

  • Lead and manage a team of community nurses and HCAs, ensuring the delivery of safe and effective patient care.
  • Develop and implement strategies to improve patient outcomes and service delivery.
  • Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
  • Manage budgets and resources to ensure efficient use of resources.
  • Ensure compliance with regulatory requirements and organizational policies.

Requirements:

  • At least 3 years of experience in a senior clinical role, with experience of managing a team of staff, including caseload management.
  • Proven skills in leading, managing, and developing teams.
  • Relevant clinical experience and competence in community nursing skills.
  • Experience of inter-agency and partnership working.
  • Experience in challenging clinical practice and leading in the development of new pathways of care and delivering best practice.

Qualifications:

  • Registered level 1 Nurse.
  • Teaching/Assessing course, such as mentorship module or equivalent experience.
  • Recognized Management qualification/course, ILM course, or leadership, management modules, core management study days, in-house courses (HR, budgets, appraisals, staff performance, sickness).

Knowledge:

  • Knowledge of the Principles of Governance, particularly in relation to their service area.
  • Knowledge of National Documents relating to District Nursing and Primary Care.
  • Knowledge of working between Acute and Primary Care settings.
  • Knowledge of Chronic Disease Management.
  • Knowledge of change management and service development.
  • Knowledge of clinical audit.
