As a key member of our community nursing team, you will be responsible for leading and managing a team of community nurses and healthcare assistants to provide high-quality patient care and services.
Key Responsibilities
- Lead and manage a team of community nurses and healthcare assistants to deliver high-quality patient care and services.
- Develop and implement effective care plans and strategies to meet the needs of patients and communities.
- Collaborate with other healthcare professionals and organizations to ensure seamless care coordination and delivery.
- Monitor and evaluate the quality of care and services provided, identifying areas for improvement and implementing changes as needed.
- Manage budgets and resources to ensure efficient and effective use of resources.
- Provide clinical leadership and guidance to team members, promoting best practices and evidence-based care.
- Develop and maintain relationships with patients, families, and community members to ensure patient-centered care and services.
- Stay up-to-date with relevant laws, regulations, and industry developments to ensure compliance and best practices.
Requirements
- Registered level 1 Nurse with a strong background in community nursing.
- At least 3 years of experience in a senior clinical role with experience of managing a team of staff.
- Proven skills in leading, managing, and developing teams.
- Relevant clinical experience and competence in community nursing skills.
- Experience of inter-agency and partnership working.
- Experience in challenging clinical practice and leading in the development of new pathways of care and delivering best practice.
- Experience of community working, or an understanding of community working.
- Experience in carrying out clinical audit and risk assessment.
