District Nurse Team Manager

Job Summary:
Livewell Southwest is seeking an experienced District Nurse Team Manager to lead our community nursing team in Tavistock. As a key member of our team, you will be responsible for managing a team of community nurses and HCAs, providing clinical leadership, and ensuring the delivery of high-quality patient care.
Main Responsibilities:
- Lead a team of community nurses and HCAs, providing clinical guidance and support.
- Develop and implement care plans, ensuring patient-centered care and effective communication with multidisciplinary teams.
- Manage budgets, resources, and staff, ensuring efficient use of resources and effective team management.
- Collaborate with other healthcare professionals, including GPs, physiotherapists, and social workers, to ensure seamless care delivery.
- Monitor and maintain accurate records, adhering to organizational policies and NMC guidelines.
- Participate in clinical audit and risk assessment, identifying areas for improvement and implementing changes.
- Develop and maintain a positive and supportive team culture, promoting staff well-being and development.

Requirements:
- At least 3 years of experience in a senior clinical role, with experience of managing a team of staff, including caseload management
- Proven skills in leading, managing, and developing teams
- Relevant clinical experience and competence in community nursing skills
- Experience of inter-agency and partnership working
- Experience in challenging clinical practice and leading in the development of new pathways of care and delivering best practice
- Experience of community working or an understanding of community working
- Experience in carrying out clinical audit and risk assessment
Qualifications:
- Registered level 1 Nurse
- Teaching/Assessing course, such as mentorship module or equivalent experience
- Recognized Management qualification/course, ILM course, or leadership, management modules, core management study days, in-house courses (HR, budgets, appraisals, staff performance, sickness)
- Ability to travel across the locality, car driver
Desirable:
- Specialist qualification in District Nursing or postgraduate diploma/degree-level study related to health
