Job Title: District Nurse Team Manager

At Livewell Southwest, we are seeking an experienced and skilled District Nurse Team Manager to join our team. As a key member of our community nursing team, you will be responsible for leading and managing a team of community nurses and healthcare assistants to deliver high-quality patient care.

Key Responsibilities:
  • Lead and manage a team of community nurses and healthcare assistants to deliver high-quality patient care.
  • Develop and implement effective care plans to meet the needs of patients and their families.
  • Collaborate with other healthcare professionals to ensure seamless care delivery.
  • Monitor and evaluate the quality of care provided by the team and implement improvements as needed.
  • Manage budgets and resources to ensure efficient use of resources.
  • Recruit, train, and develop staff to ensure the team has the necessary skills and knowledge to deliver high-quality care.
Requirements:
  • Registered Nurse with a valid NMC registration.
  • At least 3 years of experience in a senior clinical role, preferably in community nursing.
  • Proven leadership and management skills, with experience of managing a team of staff.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and other healthcare professionals.
  • Ability to work autonomously and as part of a team, with a flexible approach to working hours.
