Job Title: District Nurse Team Manager

Job Summary:

Livewell Southwest is seeking an experienced District Nurse Team Manager to join our team in Tavistock. As a key member of our community nursing team, you will be responsible for leading a team of community nurses and healthcare assistants, providing high-quality patient care and promoting health and wellbeing in our community.

Main Responsibilities:

  • Lead a team of community nurses and healthcare assistants, providing clinical guidance and support as needed.
  • Develop and implement care plans that meet the needs of our patients, working closely with other healthcare professionals and community organizations.
  • Manage and coordinate the work of the team, ensuring that patients receive timely and effective care.
  • Collaborate with other healthcare teams and organizations to promote health and wellbeing in our community.
  • Contribute to the development and implementation of policies and procedures that promote best practice in community nursing.

Requirements:

  • Registered nurse with a valid NMC registration.
  • Experience of leading a team of healthcare professionals.
  • Strong communication and interpersonal skills.
  • Ability to work autonomously and as part of a team.
  • Knowledge of community nursing and healthcare policies and procedures.
