Job Title: District Nurse Team Manager

Job Summary:

Livewell Southwest is seeking an experienced District Nurse Team Manager to lead our community nursing team in Plymouth, South Hams, and West Devon. As a key member of our team, you will be responsible for the operational management of the team, ensuring high-quality patient care and excellent service delivery.

Main Responsibilities:

  • Lead and manage a team of community nurses and healthcare assistants
  • Develop and implement effective care plans and strategies to improve patient outcomes
  • Collaborate with other healthcare professionals and stakeholders to ensure seamless care delivery
  • Monitor and evaluate team performance, identifying areas for improvement and implementing changes as needed
  • Ensure compliance with organizational policies, procedures, and regulatory requirements

Requirements:

  • Registered Nurse with a valid NMC registration
  • At least 3 years of experience in a senior clinical role, preferably in community nursing
  • Proven leadership and management skills, with experience of leading and managing teams
  • Excellent communication and interpersonal skills
  • Ability to work autonomously and as part of a team
