About the Role

Cornwall Partnership NHS Foundation Trust is seeking a Community Dementia Care Specialist to join their team. The successful candidate will be responsible for working with individuals living with dementia and those experiencing mental health needs.

The ideal candidate will have a professional qualification in RMN, current registration with the NMC, and postgraduate experience in psychological therapy, such as CBT. They will also possess substantial experience of managing a busy caseload of clients with dementia and mental health problems, along with the ability to provide clinical supervision.

The role involves working closely with statutory and non-statutory agencies to develop and deliver personalized care under the Care Programme Approach.

We offer a competitive salary of £34,382 - £43,041 per annum, depending on experience, and a comprehensive benefits package.

Main Responsibilities

  • Work with individuals living with dementia and those experiencing mental health needs.
  • Develop and deliver personalized care under the Care Programme Approach.
  • Manage a busy caseload of clients with dementia and mental health problems.

Person Specification

  • Professional qualification in RMN.
