Complex Care Manager

Leaders in Care invite applications from experienced Paediatric Nurses for the position of Complex Care Manager.
About the Role:This challenging yet rewarding role offers the chance to work closely with multidisciplinary teams, manage complex care packages, and develop meaningful relationships with clients and families.
Main Responsibilities:- Creating and delivering tailored care plans for individuals with complex needs
- Leading multidisciplinary teams to achieve seamless care delivery
- Providing regular support and assessment to clients within your caseload
- Establishing and maintaining effective relationships with clients, families, and community teams
- Working collaboratively with CHC nurses and CCG commissioners to create bespoke care packages
- Valid RSCN qualification with at least 2 years' experience in the UK
- Strong leadership and communication skills for effective team management
- Up-to-date knowledge of tracheostomy care and ventilator management
- Excellent communication and interpersonal skills
- Full driving licence
- A highly competitive salary of up to £50,000 per annum, dependent on experience
