About the Role
The Care Coordinator will be an integral part of the North Kerrier East Primary Care Network (PCN) Health and Wellbeing team, providing vital support to patients navigating the healthcare system.
Main Responsibilities
- Work closely with clinical teams to identify patients with complex needs and develop personalized care plans.
- Coordinate care between GPs, practice nurses, clinical pharmacists, physiotherapists, mental health practitioners, and health and wellbeing coaches.
- Record patient contacts and work on the clinical system against the patient record.
- Support patients and their families in managing their care, including signposting to other services.
- Network with key organizations to ensure seamless care transitions.
- Perform general administrative duties to support the PCN Business Manager and team.
