Community Healthcare Specialist

About the RoleWe are seeking a highly skilled and experienced Homefirst Assessor to join our team in West Cornwall. As a key member of our integrated community team, you will play a vital role in supporting the intermediate care needs of our local population.
Your primary responsibilities will include assessment, planning, and implementation of care, as well as working collaboratively with multidisciplinary teams to provide high-quality patient outcomes.
Key Responsibilities- Conduct thorough assessments of patients' needs and develop personalized care plans
- Work closely with community and acute hospitals, Adult Social Care, and Voluntary Services to ensure seamless care delivery
- Provide evidence-based treatment plans for adults within the catchment area, either in their own homes or nursing and care homes
To be successful in this role, you will need:
- A degree or diploma in Occupational Therapy, Physiotherapy, Nursing
- HCPC registration (or equivalent)
- Substantial post-registration clinical experience in a similar role
- Extensive experience of working within a multidisciplinary team
